For most of its life, Medisafe positioned itself as the free medication-reminder app. That changed at the start of 2026. The free tier now hard-caps users at two active medications. Adding a third triggers a Medisafe Premium paywall — a monthly or annual subscription on top of whatever you're already paying for the medications themselves.
For people managing chronic conditions — hypertension, diabetes, thyroid, depression, autoimmune — two medications isn't even the starting point. The average patient over 65 takes 4–5 prescription medications, and people with two or more chronic conditions often manage 7 or more. A 2-med cap is essentially a "please upgrade" wall by the third pill bottle.
For caregivers managing a parent's medications, the cap is even sharper: most aging-parent regimens involve 5–8 medications minimum. The new free tier is unusable for the audience Medisafe originally served best.

How to migrate from Medisafe to PillRem in 10 minutes
You can't export medication data out of Medisafe in any structured format. The migration is manual — but it's also genuinely fast.
- Open Medisafe and take screenshots of each medication's name, dosage, and schedule. Or just keep both apps open side-by-side.
- Install PillRem from the App Store (free, no account required to start).
- Run through onboarding once — name, primary profile, notification permission. About 90 seconds.
- Tap "+ Add medication" on the home screen and add each medication. Each one takes about a minute — eight short steps with friendly defaults.
- Set your reminder windows in Settings → Notifications (when does "morning" mean for you?). PillRem uses those windows to group reminders sensibly.
- Pause Medisafe for a day or two. Once you're confident PillRem is firing reminders at the right times, delete Medisafe.
If you're moving multiple family members, do them in order of complexity — your own simple regimen first, then a parent's longer list. The eight-step flow is identical for everyone; you'll be on autopilot by the third medication.

Being honest about gaps matters. There are a few things Medisafe does that PillRem deliberately chose not to do.
Medisafe Medfriend alerts. Medisafe lets you nominate a friend or relative who gets a push notification if you miss a critical dose. PillRem has its own version called Medfriends — trusted contacts you can list inside the app — but we don't currently push them automatic missed-dose alerts. That's a planned 2026 feature.
Cross-device live sync. Medisafe syncs your data live across phone and tablet. PillRem is currently single-device by design (your iPhone's iCloud Device Backup carries the data when you restore to a new phone, but it's not live sync across multiple devices). Cross-device sync is on the roadmap and being engineered with privacy-first defaults.
Pharmacy integration. Medisafe partners with several US pharmacy chains for refill ordering. PillRem doesn't. We will notify you when your stock is low so you can phone your pharmacy or use the chain's own app to reorder.
